Transcript Request Form

Unofficial LAIAD transcripts may be mailed directly to the student free of charge, one per semester. Official transcripts must be stamped, signed and mailed directly by LAIAD. Official transcript fee is $15 per mailing address - two copies will be included with each mailing, unless requested otherwise. Transcripts are typically sent 4-5 days after receipt of payment.
